Making a Point: FDA Conditions

The FDA has set strict conditions in furthering their "insurance" of making sure their decision on lifting the ban on silicone implants was the right decision. Many may feel the conditions are tough, but the FDA is doing the right thing. Prior to the ban, the pressure of the media and society to bring down silicone implants was immense, and the FDA is doing what it can to make sure that safety and prevention is carried out as a primary concern.

The following was taken from the FDA:

The FDA approved the silicone gel-filled breast implants with a number of conditions, including requiring each company to

conduct a large post approval study
continue its core study through 10 years
conduct a focus group study of the patient labeling
continue laboratory studies to further characterize types of device failure
track each implant in the event, for example, that health professionals and patients need to be notified of updated product information.

The post approval studies will continue to gather information about the safety and effectiveness of the implants. Information will be collected about rates of local complications, rates of connective tissue disease and its signs and symptoms, rates of neurological disease and its signs and symptoms, potential effects on offspring of women with breast implants, potential effects on reproduction and lactation, rates of cancer, rates of suicide, potential interference of breast implants with mammography, and MRI compliance and rupture rates. The FDA will closely monitor the post approval studies. The agency anticipates that data from the studies will provide important information for patients and physicians, and may lead to improvements in device labeling.

Those of you considering Silicone Implants may find that these restrictions and conditions to be a bit imposing, but keep in mind, it is with your health and safety in mind. Not to mention that your participation will provide valuable data and feedback to the future of silicone implants and the women who will choose to accept them.

An Important Message from the FDA

The FDA is very careful to list and provide very important information regarding the risks and benefits of silicone gel-filled breast implants. These are very important factors women should consider:

Breast implants are not lifetime devices, and a woman will likely need additional surgeries on her breast at least once over her lifetime.
Many of the changes to a woman's breast after implantation are irreversible.
Rupture of a silicone gel-filled breast implant is most often silent, which means that usually neither the woman nor her surgeon will know that her implants have ruptured.
A woman will need regular screening Magnetic Resonance Imaging (MRI) examinations over her lifetime to determine whether silent rupture has occurred.

The FDA continues to state that the MRI's as mentioned in an earlier post, over time will cost more than the initial surgery performed, however it is vital for women to take part in these studies so that silicone implants can continue to stay on the market for the future.

Silicone implants may look and feel much more realistic but they also come with a heavier price tag. If you are able to make this financial commitment make sure to also commit to the ongoing study. It only hurts the community, the women and future patients if the study is done in any manner of poor participation. Make sure you do your part. Silicone Follow Up?

When the FDA approved silicone implants they implied strict screening on the patients and a few conditions on the manufacturers, including conducting studies and following up of a projected 40,000 women over the next 10 years.

Included in this follow up procedure protocol are MRI scans that have to be done routinely. Mainly to detect any signs of implant rupture - which was led to the banning of the silicone implants in the first place.

However, the cost of the MRI scans are expensive, about $1000 and the fact that this has to be done more than once may deter women from considering silicone implants. The manufacturers don't plan on footing the bill, neither do the doctors and quite possibly the insurance companies. So who pays for the MRI's?

The patients do.

The advantage to this is that a patient receives an MRI scan, which is beneficial overall to their health. The downside is the cost and frequency of the scan. Most will argue that those seeking silicone implants are financially able to afford the MRI's as silicone implants do cost more. But with easy access to financing the silicone implants may be considered less if budgeting is a key issue.

Also add to the fact that patients have to go through a 60 page booklet on risks and considerations and fill out more paperwork than a patient would getting saline implants.

However, silicone implants ultimately look and feel more real, and to many patients, that alone is worth the work. When it comes to the body, budgeting may not always be the wisest thing, many times, as the saying goes, "You get what you pay for" and it couldn't less true when it comes to breast augmentation surgeons and silicone implants.

Natural Breast Implants?

Sooner than you think. But does that mean the end of the road for silicone and saline implants?

Unlikely.

Scientists in Japan have developed a way to use fat cells from a woman's own tissue combined with stem cells to create an implantable tissue that looks and feels smoother than current silicone and saline implants.

Although the procedure is still being tested in Japan and Europe the results look promising. There needs to be an established set of long term data, such as side effects and durability, but time will tell.

Whether or not testing will begin in the United States is up to the FDA.

Oddly enough the procedure is based off of a process developed by a California firm using fat tissue and stem cells. The problem with implanting fat into other parts of the body is that the fat tissue without a blood supply eventually dies off. The use of stem cells promotes the growth of blood vessels to fuel the fat tissue, keeping the fat cells alive.

This process also is being tested in other types of tissue replacement procedures and may eventually help with curing of degenerative diseases and or accident victims that lose parts of their body tissue.

The cosmetic use of this process was started by Dr. Kotaro Yoshimura who is a surgeon at Tokyo University who said, “I believe that within five years my procedure will be available as plastic surgery and that it will prove very popular."

The main drawback to this process is the required amount of fat tissue needed for the implant, precisely twice the amount for a single implant which can be a problem for women with small amounts of fat tissue and also a problem for those wanting larger implants.

Of the 39 patients he's tested this procedure on he claims that there have been no major problems yet.

Gummy Bears: Cohesive Gel Implants

Despite the recent approval of silicone implants by the FDA, many women are still skeptical about getting silicone implants.

There may be an alternative however. Cohesive gel implants - or gummy bear implants as they are called due to their structure have been shown to be safer and reduce the risk of connective tissue diseases related to silicone implant ruptures.

They're called gummy bears because when they are split or ruptured, they retain the shape, much like a gummy bear or a fruit snack would if you were to cut them into pieces, resulting in no leakage of silicone into the system.

Cohesive gel implants also feel more like natural breasts compared to saline filled implants.

So if you're still not sure on whether or not to pursue silicone gel implants, keep cohesive gel implants in mind, and also remember that there is no conclusive evidence that the rupturing of silicone breast implants directly leads to connective tissue diseases and any other complications that have been associated with silicone implants in the past.

Silicone Implant Impact

Jeff Carlton from the Associated Press recently reported on the effect the recent approval of silicone implants by the FDA had on the amount of surgeries doctors are getting.

In an interview with a doctor, the doctor reports that many women are considering replacing their old saline implants with new Silicone implants and that women who were also awaiting the approval are starting to make appointments to get new silicone implant augmentations.

Mentor Corp., who specializes in the manufacturing of saline and silicone implants doesn't expect an increase in production roll out, but does expect to change the ratio of which implants are produced; meaning fewer saline production and increased silicone production.

Industry experts say that women from around the world choose silicone over saline 80-90 percent of the time. Doctors and patients alike agree, silicone implants look and feel more natural.

Many are cautious of the health and safety concerns that come with silicone, but many studies suggest that there is no conclusive and direct evidence that silicone implants are attributed to connective tissue diseases. The 14-year ban by the FDA was done to extensively study the effects.

Breast implants can serve one or more of a number of purposes; breast cancer victims can use breast implants for reconstructive purposes after mastectomy, or women with asymmetrical breasts may use a single breast implant to balance the difference in size.

Following pregnancy, many women opt for breast implant surgery to correct reductions in breast size, resulting from fluctuating hormones. And of course many breast implant candidates are simply interested in creating a more flattering contour.

Since 1992, silicone gel-filled implants have been available only to breast reconstruction patients and patients undergoing breast revision surgery. In order to receive silicone, women must be enrolled in clinical trials.

It is worth explaining the differences between a silicone gel-filled and a saline-filled breast implant, which is an alternative to the silicone gel implant. The outer shell of both silicone and saline implants is made of a solid silicone material. Solid silicone is widely used in implantable medical devices. In contrast, silicone gel implants are filled with silicone gel, which is a semi-solid. Saline-filled implants are filled with the same kind of salt water that is used in I.V. fluids.
